A 67-year-old man with abdominal complaints. He appears stoic and speaks worriedly, and his face is very expressive but restrained. It started a day after they went out for dinner, Indonesian cuisine. He has severe abdominal pain, with watery diarrhea. He experiences significant pain in the lower abdomen, which is slightly swollen. He has sharp pain for an hour, followed by sour stools. Last night he sweated profusely, soaking the mattress. He continues to work, being self-employed in construction, doing a lot of heavy lifting. He has little appetite, only managing a sandwich which he finds hard to digest. Normally, he eats well and is a 'coffee man.' He never sees a doctor and doesn’t take medications because he doesn't trust them, but he regularly visits a chiropractor for his back. Last year, he fell backwards and couldn't walk for a while. At home, there are worries; it's restless, he says this fighting tears. Recently, he has been very emotional, for about a year now, since their son emigrated to Indonesia. They used to work together a lot, and in two weeks, they are going to visit him. His son left due to all the strange (COVID-19) regulations and because construction work was declining. His tongue is pale and indented.
