Spigelia anthelmia

Case

Woman, 48 years old.
Headache stinging(2) and boring(2), especially behind the eyes. Often occurs around five o'clock in the morning. She has had this headache since she was twenty.
Chronic iridocyclitis* (since 1984), over the years alternating between right and left; also headaches. HLA-positive and familliar occurring. She is followed intensively by the ophthalmologist; as medication atropine and corticosteroid drops.
The iridocyclitis feels patiente 24 hours in advance: pain and swelling of the eyes. After that, the pain becomes very intense: very (3), feeling of tapping (2)* and stinging* (2). Modalities: < bending forward (2)*, > lying with head high*, > eyes closed (pt is a member of the Blind Library), < when patient "goes to it, works for something" *. She says: "That's my character: I'm going for it and I'm perfectionist*, especially not petty".
But you show quietly? Yes, that's how I am when I work hard*..! I wake up at 5 a.m. and immediately* I think clearly. Sometimes I deliberately pick up a problem, other times I have to think inescapably (!). I stay lying down until 6 a.m. so as not to wake up my husband.
Other complaints: Wounds infect quickly. She has had a beep in her ears since an attack of iritis with flu.
Previous history: she has had many back problems (no Bechterew). Also a period with frequent cystitis (suddenly and violently; after two hours the patient couldn't leave the toilet anymore because of the heavy pressure. She then had tears in the eyes of the burning pain in the uretra*).
About her character (see above) she tells us that she is very sensitive to rejection. This was very important a few times. Patiente and her husband worked in the tropics for a long time and fully taught themselves the Thai culture. This was not accepted by their colleagues. Later in the Netherlands her husband did not get a job for 4 years and to their amazement and amazement they were no longer sent to the tropics. Possibly this was due to some 'peculiar traits' and the 'miraculous youth' of her husband. Patient became very doubtful about her husband and no longer trusted him; she had the feeling that she was responsible for everything and had to keep an eye on everything*. Her sensitivity to rejection was greatly enhanced. She felt that they were being abandoned from all sides and that she was alone in dealing with the injustice*.
Diet: Like cheese (3), French cheese (2), fruit, heavy brown bread, good glass of wine (2).
Aversion to coffee (3), pork (2), meat, fat, candy.
Intolerance to onions, Brussels sprouts.
Temperature: a bit chilly and sensitive to draughts.
Sleep quite well until 5 am, then wake up ready.

Second consultation. The patient has a clear reaction to the prescribed medicine: she has a great snot nose for three days and for 14 days she doesn't tolerate wine at all; it gives her a headache. The eyes are calm and feel good; the ophthalmologist reduced the dosage of eye drops but saw new problems: folds in the retina that pull the vitreous. But she still has the headache now and then. The squeak in the ears is also often a nuisance. Her energy was pretty good and she went well beyond her limits. Still awake at 5 o'clock.
Third consultation. Patiente is not dissatisfied, but it is far from over. The right eye is often red when waking up, no inflammation. The ophthalmologist didn't see much improvement. But the patient was very worried about last consultation and not at all anymore!
She got very tired ten days after the second consultation. The headache still occurs once in a while and is then severe.
Patiente calls after two weeks that it doesn't improve, not worse either.
Considering the pretty good improvement but the stagnation I decide to give a closely related drug.
Fourth consultation. Eye drops further reduced, goes well, also the retina is stable. The squeak in the ears is slightly improved and so is the headache.
Fifth consultation. Good. Had iridocyclitis once but it was over remarkably quickly. She has learned to take gas back (!) and that helps. Even her perfectionism is no longer so compelling: when her daughter moved, she could let a lot more happen without wanting to do everything herself.

Afterwards I saw the patient a few more times. The iridocyclitis remained calm and the ophthalmologist was very satisfied.
For fatigue, fear of rejection and increasing weight I gave twice more Calcium sulphuricum.
